This session takes a look at the decision behind the partnership and provides practical strategies that will help you revision, re-strategize, and receive the benefits of increased access and seamless transition.<\/span><\/p>\n<p><span style=\"color: #000000;\">Workshop goals and objectives:<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">To assist attendee\u2019s in assessing the value and commitment of partnering with the VA entities in their community.<\/span><\/li>\n<li><span style=\"color: #000000;\">To encourage community hospices to view Hospice Veteran Partnership as more than a marketing opportunity.<\/span><\/li>\n<li><span style=\"color: #000000;\">To develop reciprocal relationships with VA entities that allow for seamless access and reimbursement to and for services.<\/span><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #000000;\"><a href=\"http:\/\/floridahospices.org\/forum\/wp-content\/uploads\/2012\/12\/photo_Antoni1.jpg\"><span style=\"color: #000000;\"><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-758 alignleft\" style=\"border: 3px solid white;\" title=\"Charles Antoni\" src=\"http:\/\/floridahospices.org\/forum\/wp-content\/uploads\/2012\/12\/photo_Antoni1-255x300.jpg\" alt=\"\" width=\"179\" height=\"210\" srcset=\"https:\/\/floridahospices.org\/forum\/wp-content\/uploads\/2012\/12\/photo_Antoni1-255x300.jpg 255w, https:\/\/floridahospices.org\/forum\/wp-content\/uploads\/2012\/12\/photo_Antoni1.jpg 310w\" sizes=\"auto, (max-width: 179px) 100vw, 179px\" \/><\/span><\/a><strong><em>Speaker: Charles Antoni, ACHP-SW, Palliative Care Coordinator, Veterans Administration<\/em><\/strong><\/span><\/p>\n<div><span style=\"color: #000000;\">Charlie Antoni, is a Certified Hospice\/Palliative Care Social Worker with 15 plus years of community hospice and 3 plus years as Coordinator of Palliative Care Consult Team for the Orlando CA Medical Center. \u00a0Charlie is the recipient of numerous awards including the Gold Key National Honor Society, Phi Alpha National Social Work Honor Society, and the 2006 recipient of NHPCO\u2019s Heart of Hospice Award. \u00a0Charlie has been active in the Hospice Veterans Partnership since 2003 and currently serves on the Advisory Board for the joint VA\/NHPCO \u201cWe Honor Veterans\u201d initiative. \u00a0<\/span><\/div>\n<div><\/div>\n<div><span style=\"color: #000000;\">He has presented at local, state and National Conferences on a diversity of topics including Advance Directives, Cultural Diversity, Community Outreach with special populations, issues of aging and end of life, and the special needs of Veterans facing the challenges of life limiting illness. \u00a0He is a member of the OVAMC clinical ethics consult team. \u00a0He has completed both the End of Life Nurse Education (ELNEC) and Education in Palliative and End of Life Care (EPEC) Train the Trainer courses. \u00a0Charlie is a Veteran of the USAF 1966-1970.<\/span><\/div>\n","protected":false},"excerpt":{"rendered":"<p>The Hospice Veteran Partnership has been in existence for a number of years with limited success.
