This professional demonstrates peer leadership and exemplifies the provision of the highest quality of hospice and palliative care. Whether a nurse, social worker, chaplain, aides or therapist or other clinician, the Excellence in Interdisciplinary Practice winner is an example of the finest care hospice provides.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Eligibility Requirements<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-medium-font-size\">Individuals must be nominated by their hospice (cannot self-nominate).<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Nominations must be approved by the department manager and the agency CEO (or designee).<\/li>\n\n\n\n<li class=\"has-medium-font-size\">The nominator is responsible for soliciting and compiling all required nomination materials and for submitting the completed nomination forms. The submission of supporting materials is encouraged, but not required.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Nominees may not be currently serving on the FHPCA Awards of Excellence Subcommittee.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Nominators may be asked to participate in a telephone interview with the Awards of Excellence Selection.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><strong>Nomination Questions\/Materials<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-medium-font-size\">Describe the employees\u2019 outstanding achievements in relation to the patients and families<\/li>\n\n\n\n<li class=\"has-ast-global-color-1-color has-text-color has-link-color has-medium-font-size wp-elements-1543a2ec1bd06f99315be3d1125b80c2\">Describe the employees\u2019 contribution and addition to the team.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Provide at least two examples of the nominee\u2019s leadership skills that set them apart from others.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Please write 2\u20133 paragraphs on why this person should win this award.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Please include at least one digital photo of just the nominee.<\/li>\n<\/ul>\n<\/div>\n","protected":false},"excerpt":{"rendered":"<p>The Excellence in Interdisciplinary Practice Award The Excellence in Interdisciplinary Practice Award recognizes a person who has attained outstanding achievements in the direct provision of care to patients and families.
