BIG BEND HOSPICE, INC
Big Bend Hospice is a private non-profit organization which has served patients with a life limiting illness and their families in the eight counties of the Big Bend since 1983.    Big Bend Hospice provides physical, emotional, spiritual and practical care specifically tailored for each family.   Together with the patients own physician, the Hospice team of nurses, family support counselors, home health aides, music therapists, chaplains, and volunteers provide expert pain and symptom control.  Big Bend Hospice remains with each family for up to a year following the death of their loved one to provide support through the grieving process.  There are grief support programs for adults, children and teens.  All of our grief support programs are available to anyone in the community regardless of whether they have used Hospice services. LPN Runner – Part Time (Saturday and Sunday, 8:00am-8:00pm).  This position is responsible for providing after hours service to ensure that the appropriate care is given to patients in a timely manner, as deemed necessary.  Current Florida LPN license required, CHPN preferred.  Minimum of 1-2 years experience in nursing, or equivalent combination of education and experience required.  Location:  Tri-county area.

Full Time (5 days per week, 7:00am-3:00pm, weekend availability required).  This position, under the supervision of an RN, is responsible for providing personal care to assigned patients, based on the home Health Aide Plan of Care as developed and updated by RN.  This includes bathing and assisting with ADL’s as appropriate.  Current Florida CNA certificate required, CHPNA certification preferred.  Minimum of one year experience as a CNA required, home health experience preferred.  Location:  Tri-county area.

IPU CNA – Part Time (25 hours per week, 12 hour shifts, days vary, 7:00am-7:00pm).  This position, under the supervision of an RN, is responsible for providing personal care to assigned patients in the Inpatient Unit, based on the home Health Aide Plan of Care as developed and updated by RN.  This includes bathing and assisting with Activities of Daily Living as appropriate.  Current Florida CNA certificate required, CHPNA preferred.  Experience with Homeworks or other Health Information Management software preferred.  Minimum of 3 years experience as a CNA required, BLS, hospice and/or critical care experience preferred.

Annual Fund Coordinator – Full Time (40 hours per week 8:00am-5:00pm). 
This position is responsible for planning, organizing and managing the annual fund campaign and researching and cultivating donor relationships to solicit and/or increase giving.  Bachelor’s degree in related field preferred, or equivalent combination of education and experience required.  Minimum of 2 years experience in fundraising, non-profit development or other related field required.  Excellent written and verbal communication skills as well as excellent public speaking skills required.  Knowledge of database software, Microsoft Office Suite and Internet software (for prospecting research purposes) required,     experience with Benevon fundraising model, including tours, events and daily donor activities, preferred.  Location:  Altamonte Springs.

Director of Volunteer Services – Full Time (40 hours per week, Monday-Friday, 8:30am-5:00pm, some nights/weekends may be required).  This position is responsible for the development and implementation of an effective volunteer services program for Hospice of the Comforter.  Additional responsibilities include: ensuring proper coordination of the volunteer program as stated in the Medicare Conditions, establishing goals and objectives and tracking the overall success of the marketing, recruitment, selection and retention of volunteers, developing and coordinating community awareness campaigns and marketing activities to introduce Hospice of the Comforter to the community and prospective volunteers, and supervisory responsibilities for a staff of 7.  Bachelor’s degree in a related field required, Masters degree preferred.  Minimum of 5 years volunteer services or related field and 3 years supervisory experience required.  Experience in a healthcare or hospice organizations is preferred.  Must be willing to travel within the tri-county area to promote Hospice of the Comforter to the community a minimum of 10% per year.

Chaplin
PRN availability. Coordinate with the Triage team, provides spiritual care services for patients/families during the non-business hours on weeknights and weekends. Provides spiritual counsel for patients and/or families in spiritual distress and support for families at the time of a patient's death. Contributes to the smooth function of services for a 24 hour/7day a week hospice care program.
Master's Degree from an accredited seminary or school of theology.
Minimum five (5) years experience in pastoral care, at lease one (1) year of which was in health care setting. Psychosocial Specialist
PRN availability. Acts as part of the interdisciplinary care team and manages social service needs of a caseload of patients and families.
Coordinates all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement. Contributes to the smooth function. MSW from an accredited school of Social Work and three (3) years direct service experience, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs with the team and community at large.

Associate Medical Director
Must be a Florida Licensed Physician who has experience in the practice of inpatient hospital medicine.  S/he will provide physician services to patients of Community Hospice, shall be a member of the Interdisciplinary Team (IDT) of a Hospice In-Patient facility and work closely with the IDT members, patient/family and the primary or referring physician to ensure quality care for the patient and family.  Typical qualifications would be equivalent to Doctor of Medicine or Osteopathy degree from an accredited medical school with a current Florida license, must be eligible for, or have membership in good standing on the medical staff of at least one of the accredited hospitals in the Community Hospice service area., Hospice and Palliative Medicine Certification by the American Board of Hospice and Palliative Medicine is desirable within 24 months of hire (if not already certified), experience with oncology, home health or hospice care delivery is desirable.

Advanced Registered Nurse Practitioner
This position will provide independent primary care to hospice patients in the inpatient setting for their general medical illnesses.  Though these are patients with terminal illnesses, the focus of this provider will be to render for unassociated medical illnesses separate from their terminal diagnosis.  Masters Degree with PNP certification, current state of Florida ARNP license, strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community, ability to maintain quality, safety and/or infection control standards, ability to work flexible hours when needed.

This position will plan, direct and manage the activities of the information technology department to ensure alignment with overall business needs, strategic plan, budget and policies of the organization.  BS degree in Computer Science and/or a minimum of 7 years work experience in managing one or more major information processing functions, Masters degree preferred, 3-4 years of management and staff supervisory experience, comprehensive knowledge of hardware, software, networking, project and Process Management skills and techniques.

Major Gifts Officer
This position will develop relationships with donors and volunteers that result in visibility and financial resources to support our mission and services. Plan and conduct, with the support of others, specific campaigns for major gifts.  Directly cultivate and solicit major gifts from individuals and organizations.  Identify, cultivate and secure grant opportunities.  Bachelor's Degree required plus 7 years of related fundraising experience including proven track record of direct cultivation and solicitation of major gifts from individuals and organizations. Intake Facilitators
Call center representatives will perform a wide range of activities pertaining to the accepting and processing referrals and dispatching Admission RNs. Coordinates assessment appointments and personnel; uses independent judgment within the framework of established policies and services.  Minimum requirement of a high school diploma; AA or Bachelor's degree preferred, 6 months experience in medical call center environment, or in a health care setting, good written and verbal communication skills with strong customer service orientation, computer knowledge (experience with patient/client data/software, computerized voicemail and paging experience preferred).

HomeCare Manager
The Homecare Manager utilizes management skills to facilitate provision of care in HomeCare.  Under physician's orders, per the Physician's Plan of Treatment, the manager is responsible for identifying and coordinating patient/family needs and for providing palliative, preventative, and rehabilitative care to the patient/family unit. The manager utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. The manager is a mentor, teacher,  and preceptor to new staff and team members while providing home health care. The manager participates in quality management initiatives and financial management for the certified and non-certified patients.

Requirements:

1. Current Florida Registered Nurse license, BSN preferred.

The Home Health Case Manager provides skilled services to patients in their homes. The Home Health Case Manager is a mentor, teacher and preceptor to new staff and team members while providing home health care. Must have FL Registered Nurse License and a minimum of 2 years home care experience. Oasis competency required and computer skills a plus. Respiratory Therapist - Monday-Friday/Floating
Provides respiratory services to patients utilizing oxygen systems, bi-pap, cpap, ventilators and other hi-tech equipment. Picks up invoices and prepares equipment check on products to be delivered. Ensures proper delivery pick up address and correct services rendered. Responsible for final check on all equipment being delivered. Verifies that all equipment/products in staging area is completed and listed on invoice. Verifies that all products being delivered are in good working order, clean, adjusted and operating properly. Ensures all equipment being delivered has serial numbers if appropriate. Calls customer to inform approximate arrival time, if money is due and verifies equipment being delivered. Reports changes in deliveries to reflect proper invoicing. Ensures all deliveries/pick ups are logged appropriately and accurately. Participates in disease or case management aspects of patient care. Maintains adequate supply of mobile forms and all forms necessary to provide service and maintain documentation.

Marketing and Sales Manager - The Medical Store - Monday - Friday; 8:00A-4:30P
Position is responsible for managing the Marketing and Sales team to develop, implement and administer a Business Development and clinical sales plan with the goal of increasing the number of referrals to The Medical Store. Develops a medical equipment marketing plan, in coordination with the community awareness plan, with measurable objectives. Effectively markets assigned regions to drive sales to The Medical Store. Pursues new referral sources to include relationship/contract opportunities with managed care companies, hospitals, extended care facilities, other healthcare organizations and corporations. Selects, supervises and evaluates the sales and marketing staff. Motivates the sales and marketing staff for optimum performance through team building and acts as a mentor and resource for the staff. Performs quarterly evaluations of the sales and marketing plan and progress reports to the Vice President of Operations. Assists with the development and implementation of short and long range goals for increasing The Medical Store awareness and understanding of services among healthcare professionals to include, but not limited to, physicians and office staff, discharge planners and other hospital staff, nursing home staff and payers. Works with internal and external resources to foster positive relations on behalf of TMS with healthcare organizations, healthcare professionals and community groups. Ensures the sales and marketing plan is maintained within budget and coordinates marketing efforts of all staff and volunteers. Utilizes computer support and data management tools to increase efficiency and productivity. Coordinates the servicing of all referral sources to include coordinating inservices and other educational needs as needed and as defined by contractual agreements. Evaluates internal and external concerns and responds to the concern in a timely manner. Ensures that staff counseling and discipline is appropriate.
