Founded as “Hospice of Naples, Inc.” in 1983. We are Collier County’s original – and only – nonprofit hospice, serving our community for more than 26 years. We care for those who are terminally ill and support their friends and families through bereavement. Our Frances Georgeson Hospice House, built with community donations, is the only freestanding hospice inpatient facility in our county. As our community’s resource for grief and loss support, we hold grief workshops in Collier County schools, offer a range of no-cost bereavement support groups, work with those who have lost a pet and educate professionals and lay people on a multitude of issues surrounding end of life. We raise funds through a number of initiatives both to supplement reimbursements we receive for basic hospice care and to provide “added” services – like massage therapy, music therapy and Reiki energy treatments – we believe our patients and their families need and deserve. BIG BEND HOSPICE, INC
Big Bend Hospice is a private non-profit organization which has served patients with a life limiting illness and their families in the eight counties of the Big Bend since 1983. Big Bend Hospice provides physical, emotional, spiritual and practical care specifically tailored for each family. MSW from an accredited school of Social Work and three (3) years direct service experience, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs with the team and community at large.
Associate Medical Directors
Under the direction of the Chief Medical Officer, the Associate Medical Director shall be a physician licensed to practice medicine in the state of Florida. The Associate Medical Director has responsibility for medical direction of the hospice patient’s care and treatment, and consults and cooperates with the patient’s attending physician. The Associate Medical Director also educates community physicians and others engaged in healthcare services regarding the hospice program and its potential benefits to patients.
Requirements: Doctor of Medicine or Osteopathy degree from an accredited medical school with a current Florida license. Hospice and Palliative Medicine Certification by the American Board of Hospice and Palliative Medicine is desirable within 24 months of hire (if not already certified). Experience with oncology, home health or hospice care delivery is desirable.
Communications Specialist II
Under the immediate supervision of the Director of Communications, the Communications Specialist II - Foundation and Special Projects will be responsible for oversight of all Community Hospice Foundation communication projects, with a goal of creating broad-based, appealing and consistent messages that build donor investment in the organization. These include, but are not limited to, developing and implementing strategic collateral materials and newsletters, media relations initiatives, direct mail and donor-centered appeals (including Thrift Shop promotions and awareness), production and placement of advertising as needed, and relevant Web site-based support.
Requirements: Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. The LPN is an essential member of the patient care team. LPN POOL-Continuous Care
Duties:ONE on ONE patient care ~ Cases in Polk county, must be available to travel to Highlands/Hardee counties as well. Also, MUST be available for 12 hour shifts, mostly 8p-8a. Under physician's orders, and RN supervision, the Good Shepherd Hospice Continuous Care LPN is responsible for the observation of patient/family needs and providing palliative and supportive care to the patient/family unit. Medical Director
The Medical Director (MED) will provide physician direction and guidance to the hospice and palliative care program and staff to assure the maintenance of quality care for patients and families, serve as liaison between the program and community physicians, as well as with others engaged in health care services, serve as an educator and researcher of hospice care. Provides direct medical care as needed to patients in the home and in multiple health care settings. HPC Healthcare Facts: During 2008, 871 physicians referred patients to HPC Healthcare and 59 nursing homes and 153 assisted living facilities partnered with the organization. Provides hospice care to patients throughout Hillsborough, Polk, Highlands and Hardee counties. In 2008, HPC served 12,361 hospice patients and admitted 8,976 new hospice patients. • Current Florida LPN license with IV certification. 4-5 years nursing experience; preferably in oncology, palliative care, geriatrics, or home health care. 1-2 years hospice experience preferable. 1-2 years scheduling staff, preferable. Associate Director of Development
• Targeting, cultivating organizational and business partnerships throughout the network that enhance fundraising activities. Planning and implementing all aspects of donor cultivation, management and stewardship for assigned territory. Creating and maintaining donor profiles using the Raiser's edge software. Coordinate necessary volunteer and staff participation in appropriate events and activities.
• Bachelor's Degree required; Master’s degree preferred. Two (2) to five (5) years of demonstrated successful fundraising history essential. Director of QAPI
Manages the implementation of the Quality Assessment and Performance Improvement Corporate Plans; includes Joint Commission oversight; Patient Safety Standards; Infection Control and Risk Management Programs. Qualifications:

Corporate Social Services / Bereavement Director
This position has corporate responsibility and provides leadership for all facets of Social Services/Bereavement care delivered to HPC’s hospice patients and families. Develops, implements, and maintains policies and procedures. Sets corporate standards and assures regulatory compliance. Works with clinical leaders to ensure the highest level of Social Services/Bereavement care is being delivered and provides recommended solutions when gaps and/or issues are identified. Provides input regarding budgets as they relate to Social Services/Bereavement care, including staffing ratios, educational requirements, and compliance with discipline specific regulations and standards. Develops or refines discipline specific programs and services for the company. Qualifications: Master’s degree from an accredited school of Social Work and current license in Clinical Social Work required. (If out of state license, must acquire Florida license within 1st year of employment.) Hospice or Home Health experience preferred. Experience in program planning, in-depth knowledge in area of specialty and the ability to translate professional and organizational goals into effective programs.
Development Coordinator
Duties:The Development Coordinator is responsible for raising funds for HPC (LifePath Hospice/Good Shepherd Hospice) through annual giving programs such as special events, community events, employee campaign and special projects. The Development Coordiantor will work directly with the Corporate Annual Giving Director in the coordination, planning, cultivation, reporting and logistics for these projects. Qualifications:Undergraduate degree (or significant and substantial fund raising, marketing or public relations experience). 3 years non-profit experience, preferably in special events or memorial campaign. Excellent oral and written communication skills.
Computer experience including internet research, word processing, windows, data base management and familiarity with Raiser's Edge software. Valid Florida driver's license and full coverage auto insurance required. Ability to travel to off-site locations. Highly organized; able to effectively manage many tasks simultaneously. Works with minimal supervision and uses independent judgment.

Manager, Patient Accounting
This position provides management and leadership for non-patient care areas within the Hospice operating environment including billing and collection for Company services, financial counseling, monitoring, and special projects. Manager, Volunteer Services
Provides leadership and direction for the company's volunteer program, ensuring effective integration of the volunteer component throughout the organization and community. Evaluates volunteer service outcomes, identifies and promotes solutions. Evaluates practice standards and conducts formal and informal educational programs/experiences of company members to support Company volunteer service program. Supervises the volunteer coordinators and volunteer department secretary. Qualifications: Degree in a human service related field required. At least 2 years experience in a health care setting. Respiratory Therapist
Responsible for respiratory equipment set up and education regarding respiratory equipment to patients, families and company staff. Participates in the initial respiratory assessment process for patients enrolled in the C.O.P.D. program and makes recommendations based on this assessment. Primarily responsible for inital set up and maintenance of CPAP and BIPAP systems. Serves as a consultant to the company staff for patients with other respiratory diseases and symptoms. Qualifications:* Registered Respiratory Therapist (RRT) or Certified Respiratory Therapy Technician (CRTT) license issued by the Florida Department of Health, Able to travel to off-site locations (Must be willing to travel to all sites in the Hillsborough, Polk, Highlands and Hardee Counties), Valid Florida driver license and auto insurance per company policy. Working knowledge of home respiratory equipment and medical supplies including, but not limited to: BIPAP, CPAP, Liquid Oxygen, Suction, oxygen concentrators, nebulizer therapy. The RN oversees the collection and aggregation of performance improvement measurements for all sites within the subsidiary. The Hospice House Manager is responsible for direct oversight of all aspects of patient care provided by hospice house staff members twenty-four hours a day, seven days per week, including case management, fiscal accountability, risk, compliance, and personnel management. S/he is responsible for the coordination of care across the continuum through collaboration with other departments, patients and families. RN On Call (Weekends) Palm Coast, FL
Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing and company policies when called upon after hours. Prioritize calls, and instruct nurses to see patients as needed. Guide after hour’s staff in assessing and treating physical, psychological, psychosocial and spiritual needs. Under the direction of the Chief Medical Officer, the physician has responsibility for the medical direction of the hospice patient’s care and treatment, and consults and cooperates with the patient’s attending physician and interdisciplinary team. Requirements: Doctor of Medicine or Osteopathy degree from an accredited medical school with a current Florida license. The physician will be responsible for the clinical integrity of the program, including the oversight of clinical assessment and treatment, as well as direct patient care responsibilities. Duties include development and monitoring of clinical practice protocols, adherence to palliative care guidelines and integration of quality standards into the clinical review process and delivery of care. The physician will also provide supervision and oversight of ARNP staff and support, guidance and education for other members of the palliative care team, including RNs, social workers and clergy. Candidates must be board certified in hospice and palliative medicine. Two years experience working in a palliative care setting is preferred. Senior Staff Nurse (Evening Shift) – Woodside Hospice House – Pinellas Park, FL
Suncoast Hospice is currently seeking a Senior Staff Nurse for its Wooside Hospice House. The Senior Staff Nurse (SSN) provides support and leadership in coordinating team functions in collaboration with and support of the Patient Family Care Coordinator (PFCC), including Quality Assessment Performance Improvements, supervisory visits, and other activities as appropriate. The Senior Staff Nurse is an active, formal mentor and first line resource to the Nursing and Hospice Aide staff on the team, providing teaching, support, and guidance. Requirements:

RN TEAM MANAGERS- Full time
Head up the interdisciplinary teams managing your hospice patients. Internal office position with some on call required.
ADMISSION NURSES- Full time
Present VITAS services and philosophy to patients and families, transitioning the patient onto Hospice services. Must be willing to travel (within Collier County) to where the patient is located. Bilingual in Spanish a Plus!
LPNs- Full time; part time; PRN
Provides skilled nursing care as determined by the interdisciplinary plan of care. Teaches families and other primary care persons appropriate care techniques. Minimum two years medical/surgical, with hospice, oncology or home health experience preferred.
